COVID Policy and Proceedure

COVID-19 Liability Risk Consent
*I acknowledge the contagious nature of the Coronavirus/COVID-19.
*I further acknowledge that the Missouri RYLA Academy and the University of Central Missouri have put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.
*I further acknowledge that the Missouri RLYA Academy and the University of Central Missouri can not guarantee that my participant will not become infected with the Coronavirus/Covid-19. I understand that the risk of my participant becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of my participant or/and others, including, but not limited to, academy staff, venue staff and other RYLA participants and their families.
I voluntarily seek to allow my participant to participate in the Missouri RYLA Academy on the campus of the University of Central Missouri in the activities and services provided by the aforementioned academy and university and acknowledge that I understand that my participant is increasing their risk to exposure to the Coronavirus/COVID-19. I acknowledge that he or she must comply with all set procedures to reduce the spread while attending the academy. I attest that:
* My participant is following all CDC recommended guidelines as much as possible and limiting their exposure to the Coronavirus/COVID-19.
I hereby release and agree to hold the Missouri RYLA Academy and the University of Central Missouri University harmless from, and waive on behalf of my participant, myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to my participant, myself and/or property that may be caused by any act, or failure to act of the academy or university, or that may otherwise arise in any way in connection with any services received from the Missouri RYLA Academy. I understand that this release discharges the Missouri RYLA Academy and the University of Central Missouri from any liability or claim that my participant, myself, my heirs, or any personal representatives may have against the academy and university with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any activities and services received from the Missouri RYLA Academy and the University of Central Missouri. This liability waiver and release extends to the parent organization of Rotary International, Missouri RYLA Academy, directors, staff, board members, partners, the University of Central Missouri and its employees.
Participant COVID Symptoms Protocol *
I understand that if at any time during the Missouri RYLA Academy my student shows symptoms potentially related to COVID-19, they will be immediately removed from the group and placed in a safe, designated location on campus in isolation. I understand that as the legal guardian, I will be notified immediately by the academy director, with the expectation that I or another approved legal adult will come pick my student up as soon as possible.
